We regret to inform you that our practice has discovered a potential breach of your personal health information. We became aware of this breach on January 30, 2017. We believe that information containing your name, address, social security number, date of birth, diagnosis, etc. was disposed of improperly during the shredding of medical records over six years old, on or about January 25, 2017. We have not received any indication that any information has been accessed or used by an unauthorized individual. PLEASE NOTE THIS AFFECTS A VERY SMALL NUMBER OF PATIENTS WHO RECORDS ARE MORE THAN SIX YEARS OLD.

We are actively investigating how this breach happened and taking the proper steps to ensure that issues like these do not happen in the future. To protect against such breaches in the future, we have held employee retraining, and updated procedures for shredding records.

We apologize for the stress and worry this situation has caused you. We are committed to keeping your information safe and assure you we are doing everything possible to regain your trust in our practice. If you are concerned your records are included in this notice, please call the office and speak with me and I would be happy to check. All patients who were affected were sent a letter via USPS.
